Abstract

Atypical aneurysms, including intracranial mycotic aneurysms, dissecting aneurysms, and pseudoaneurysms, present distinct challenges for diagnosis and management. We review the pathogenesis, diagnosis, and management of these atypical aneurysms. While newer vessel wall imaging techniques promise greater sensitivity, catheter angiography is often necessary for diagnosis. Ruptured atypical aneurysms can cause significant morbidity; unfortunately risk of rupture can be difficult to predict. Endovascular and surgical intervention can preserve the underlying vessel or require parent vessel occlusion. Treatment is individualized to the patient’s clinical situation and anatomy.
